Essential Functions

  • Dedicating a minimum of 17 working hours per week will be spent providing quality direct service to participants. This includes conducting comprehensive intake assessments and co-creating individualized, strengths-based housing stability plans with young parents to ensure long-term independent living and housing retention
  • Receive incoming referrals and conduct comprehensive intakes to determine program eligibility; collaborate with with participants to develop individualized housing stability plans aimed at preventing homelessness
  • Meet with clients according to program requirements to model and practice independent living skills, including but not limited to: academic and career planning, permanent relationships/mentoring, housing/household maintenance, priority setting, money management and health and wellness
  • Ensure up-to-date paper & electronic client records in compliance with program, county, state and federal regular and private grant requirements as applicable
  • To provide youth referrals and linkages to community based services. This includes tracking follow up and status of referrals and youth progress with services as well as to foster and maintain working relationships with other agencies that come into contact or work with youth

    Minimum Qualifications
  • HS Diploma or GED
  • 1-2 years of experience working with youth and young adults
  • Driver's License

    Physical Requirements:
  • Sedentary work that primarily involves sitting/standing
  • Able to climb in and out of vehicle multiple times per day
  • Able to walk up and down stairs as necessary
  • Reach with hands and arms and sit for prolonged periods of time at a desk, working on a computer
  • Able to drive throughout the Philadelphia area
