Essential Functions of the Caseworker Specialist (IL):
- Facilitate 132 hours (3 hours a week x 44 weeks) of 1:1 Independent Living instruction with their assigned caseload working on youth-established goals in the Transition plan at the level of service determined by the Service level needs assessment.
- Spend a minimum of 35% working hours (13 hours a week x 44 weeks = 572 hours annually) providing quality in-person goal planning, case management, referrals and coordination of services, team communication, safety assessments, and attending to the general well-being of the client. This is in addition to the required independent living instruction. This includes ensuring the safety of each client assigned to them every 30 days by completing a minimum of one 30-minute face-to-face visit in the client's home and completing the formal safety assessment in CAPS. This should be done in accordance with the PA Dept. of Human Services (DHS) Code and DHS Bulletins.
- Ensure the timely & accurate completion of all required paperwork (including documentation requirements) per the AAI Paperwork Guidelines documents, the approved program description, and all YCOCYF's policies, procedures, and laws.
- Ensure timely updates to all members of the client's team per the guidelines listed in the approved program description. Clearly and concisely provide all necessary updates and recommendations to the court while maintaining a professional presence in all aspects of your work.
Minimal Qualifications for the Caseworker Specialist (IL):
- An associate's degree in Social Work or related field. If the degree is in an unrelated field then the candidate must have at least 1 year of experience in the social services field providing case management services.
- One or more years experience working with youth and young adults or providing case management services.
- Candidates must be in good standing with the York County Office of Children, Youth and Families given the close working nature of this position.
- Candidates must be highly organized and capable of independently managing their deliverables due to the nature of this work and the high level of local and state oversight. The work of this position is subject to the PA Department of Human Services' Office of Children, Youth and Families licensing requirements.
- A valid driver's license, a motor vehicle report that meets agency requirements, and reliable transportation that can safely transport clients if needed.
- Ability to work a hybrid schedule that includes evenings.
- Ability to complete the pre-employment physical and drug test requirements.
Preferred Qualifications for the Caseworker Specialist (IL):
- One to two years of experience working with adolescents.
- Knowledge of the Child Welfare system, York County resources, and service providers.
- An understanding of the barriers and challenges that youth involved in the Child Welfare system face including the need for permanency and reasons for truancy, lack of self-esteem, parent-child conflict, etc.
