SWAN Coordinator

Position: FT- SWAN Coordinator ($21hr to $26hr)

Location: Bethlehem

Essential Functions of the SWAN Coordinator:

  • Carry a 75% caseload and complete 85% or greater of all assigned SWAN units of service and ensure all services are completed in a timely and efficient manner based upon SWAN benchmarks, which are to be updated with regularity on the SWAN Portal.
  • Complete units of service according to Benchmark requirements including all outreach of records, interviews, and surveys. This includes timely and multiple follow ups on requests.
  • Provide editing feedback for assigned Child Profiles.

Minimal Qualifications of SWAN Coordinator:

  • BA/BS in social work or similar degree in human services.
  • Two years of related experience in the field.
  • Must have and maintain a valid driver's license including vehicle insurance and inspection on any vehicle used for daily work and transportation of clients. Travel is required within Northeast and Southeast Pennsylvania counties on a daily basis.
  • Display organizational skills and be highly adept at managing multiple tasks and assignments.
  • Excellent writing skills and engagement skills.

Physical Requirements of SWAN Coordinator:

  • - Sedentary work that primarily involves sitting/standing. X Constantly
  • - Light work that includes moving objects up to 20 pounds. X Occasionally
  • - Operating a motor vehicle requiring getting in and out of car several times per day, walking within the community, and entering/exiting homes that may contain stairs X Constantly
