Essential Functions of the Housing Stability Coach

  • To provide street-based outreach in Lancaster to youth who live or spend a signi?cant amount of time on the street or in other areas of exposure that place them at risk of abuse, trafficking, and exploitation.
  • Successfully engage youth/clients in developing and implementing their individual goal plan using positive youth development, trauma-informed, and culturally appropriate frameworks in the Lancaster Rapid Rehousing program.
  • Ensure the timely & accurate completion of all required paperwork per the program requirements.
  • Provide a minimum of 24 hours a week of 1-on-1 sessions/credible contacts with clients identifying barriers and connecting to resources to alleviate those barriers. A focus on harm reduction in street outreach efforts is imperative.
  • Be the primary liaison between the Coalition of Homelessness teams and Valley Youth House.

Minimum Qualifications

  • An associate's degree in Social Work or other related ?eld. If the degree is in an unrelated ?eld then the candidate must have at least 1 year of experience in the social services ?eld.
  • One or more years of experience working with youth and young adults or in case management or street outreach.
  • A valid driver's license, a motor vehicle report that meets agency requirements, and reliable transportation that can safely transport clients if needed.
  • Ability to work a hybrid schedule that includes evenings.

Preferred Qualifications

  • One to two years of experience doing street outreach or case management with individuals experiencing homelessness.
  • Knowledge of the services available in both York and Lancaster Counties.
  • An understanding of the barriers and challenges that individuals experiencing homelessness face, especially those unique to the 18-24-year-old population.
