Emergency Services Intake Case Manager

Pay rate:($22.50 - $26hr)

Essential Functions Emergency Services Intake Case Manager:

  • To Support the day-to-day operations of the Youth Designated Access Point including drop-in days, shelter placements, rotating hotline coverage and assessment schedules as assigned. 
  • To operate The Synergy Projects emergency Hotline service, screen for eligibility for services, and maintain an up-to-date list of resources and referrals.
  • To coordinate and track housing intakes and referrals including intake appointment reminders, housing applications, and communication with housing programs.
  • To complete required tracking and program documentation in a timely manner. This includes entry into local HMIS.
  • To maintain professionalism and appropriate boundaries and conduct in all settings where services may occur including, but not limited to agency vehicles, community locations, and office spaces

Qualifications Emergency Services Intake Case Manager:

  • High School Diploma or equivalent
  • 1 - 2 years of working with youth and young adults
  • Have and maintain a valid driver’s license and ability to operate agency vehicles.
  • To never have been convicted of any prohibited criminal or child abuse offense.
  • Possess sound judgment and knowledge of youth and young adults’ development.
  • Organized and highly adept at managing multiple tasks and assignments
  • Be able to work untraditional hours including nights and weekends based on the needs of the program operations

Physical Requirements:

  • Frequently is required to stand; walk; and sit for prolonged periods of time at a desk, working on a computer.
  • Light work that includes moving objects up to 20 pounds. X Occasionally
  • Medium work that includes moving objects up to 50 pounds. X Occasionally
