We are currently looking for a qualified applicant to fill the following position: FT - Case Manager ($14hr)
Job Description: Case Manager's time will be spent providing direct, in-person services according to established program time frames and protocols. To successfully engage and maintain youth and families through the treatment process.To provide effective case management services to clients in office, client homes, schools, and other settings as appropriate within the context of outreach services and according to treatment plans. To provide regular ongoing case coordination and collateral contact with other community agencies also involved in individual cases. To achieve stated treatment goals with assigned families. To represent the program in treatment conferences with other agencies in a professional manner.
Qualification: A Case Manager shall be a mature person with sound judgment and represent an appropriate role model to youth and families. Must be an individual who has never been convicted of any child abuse offenses. A Bachelor's Degree is required, training and experience in crisis intervention and one year experience in a CASSP system is mandatory for Case Managers in the Children's Behavioral Health Component.
