We are currently looking for a qualified applicant to fill the following position: FT - Case Manager ($15.50 - $18hr)
Job Duties: Under the Senior Vice President and President and CEO's authority and the Program Supervisor's supervision, the Case Manager is responsible for the contact with and treatment service to youth and families; contact with community agency personnel; and administrative and clinical documentation of services provided.
- Case Manager's time will be spent providing direct, in-person services according to established program time frames and protocols.
- To provide individual, parent, family group counseling/case management services to clients in client homes, schools, office, and other settings as appropriate within the context of outreach services and according to treatment plans.
- To complete and submit to the case file all required clinical documentation in a professional manner within established program expectations and time frames
- To complete child/adolescent and Home Based Safety Assessment and/or parent intake interviews and their subsequent dictation according to established program time frames and protocol.
- To achieve stated treatment goals with assigned youth or families as identified by program expectations.
- To provide effective services in accordance with the program service model and program expectations, including collaboration with members of the youth's treatment team.
- To identify, link, and coordinate all necessary services.
- To provide regular ongoing case coordination and collateral contact with other community agencies also involved in individual cases.
- To participate in supervision with his/her assigned supervisor according to program time frames and protocols.
- To meet at least one time per week with his/her assigned supervisor individually or as part of a team.
- To successfully engage and maintain youth and families through the treatment process.
- To follow the Incident Reporting procedure as required by federal, state, and agency practices and procedures.
- To immediately report to the supervisor 1.) Any situation in which a child, family member, or another person is at risk of imminent harm and/or 2.) A death occurs in an open case or a case open at the agency within the last 90 days.
- To immediately report to the supervisor any situation in which a client is threatening to sue Valley Youth House.
- To provide crisis response to youth and/or families in need, which may include responding by phone or going to the family home or community site.
- If the program has on-call responsibilities, the responsibilities include responding by phone within 15 minutes of the contact and going to the family home or community site if the situation cannot be resolved by phone.
- To follow the policies, procedures, and rules stated in the agency personnel policies, Code of Business ethics, employee handbook, and program procedural manual.
- The accurate and timely completion of personnel material (e.g. mileage reimbursement, time sheet, etc.).
- To support agency values and goals.
- To provide quality representation of the agency with other professional agencies or in court in a professional manner.
- To provide quality representation of services with an individual family in court when subpoenaed.
- To represent the program in treatment conferences with other agencies in a professional manner.
- Timely completion of training in accordance with program standards.
- To complete other related tasks as appropriate or assigned.
Qualification: A Case Manager shall be a mature person with sound judgment and represent an appropriate role model to youth and families. He or she must be an individual who has never been convicted of any child abuse offenses. A Bachelor's Degree in a human service discipline and one-year of CASSP experience is necessary.
